I shared my journey on why a #nephrologist is advocating for POCUS-assisted physical examination to be standard of care
I was brought to set up a Transplant and an Interventional Nephrology program in Abu Dhabi Health Services Company- SEHA - شركة أبوظبي للخدمات الصحية - صحة Dialysis Service. During my first 3 years in SEHA, I was the catalyst for and spearheaded the digitalization of the end-stage renal disease program. We bought hundreds of millions of offline annual transactions online which has had many cost benefits and brought many established 21st century kidney care best practices to UAE & establishment of a structured transplant program in SEHA Dialysis Services
But I soon realized that this ESRD-centric #kidneycare model we have inherited from the West is all unsustainable long-term & currently only possible in Abu Dhabi due to the generosity of the Government of Abu Dhabi which is paying for all this despite most of the patients being expats.
The primary work of a #nephrologist
A. Is to prevent #kidneyfailure
B. If they cannot prevent that due to the nature of the disease at least get them off dialysis by way of #kidneytransplant and
C. If they can't get them transplanted due to a lack of donors or they are not transplantable, then at least prevent dialysis patients get hospitalized
Most common cause of Kidney Failure in hospitals is "Acute Kidney Injury(AKI)". One in 5 patients in hospitals gets AKI. The most common causes of in-hospital AKI
a. is too much fluid( congestion)
b. too little fluid ( hypovolemia)
c. Obstruction
➡ Bed Side POCUS is a great tool to address this
In patients who are on dialysis, the two main reasons they get hospitalized is
a. Fluid Overload- Congestion
b. Dialysis Vascular Access dysfunction
➡ Bed Side POCUS is again a great tool to address this
Hence this advocacy of POCUS education to be embedded in Internal Medicine as Hemodynamic " volume" assessment underpins everything we do in internal medicine from shock, sepsis, cardiac failure, hepatorenal syndrome, etc uniting the language from ED 🤝 Internal Medicine 🤝 Critical Care for "Volume" assessment
With the availability of #aitools for image acquisition and quality assessment, POCUS is something we can teach all members of the multi-disciplinary team to do, improving the costs of delivering care
The median age of those ending up on dialysis in our region is around 40 years, we need to embrace all these emerging technologies to deliver care that is affordable, accessible, and in the end sustainable
